Carry it Forward provides case management with daily check-ins. This flagship project helps participants get into housing, get to medical care, have needed surgeries, and provides next steps for reaching goals.

SPOT (Shelter Prioritizing Onward Traction) works with client strengths to address challenges and build resilience; case management and advocacy toward housing and employment goals, skills-training, and steps to build positive community supports.

We offer assistance with phone calls, job and housing applications, obtaining ID and personal documents, signing up with OHP, researching resources, advocacy and help navigating housing/legal/medical/disability services, and connecting with resources to meet participants’ individual needs.

Other areas of focus include skill-building, transportation for personal errands, mental health/substance use treatment through community partners, making positive community/social connections, and pursuing personal interests/hobbies.
